Equipment Failure Forensics

Topic: "Equipment Failure Forensics"

This presentation consists of a review of several failure investigations involving electrical power system failures. Each case study includes photos of the failed equipment and a discussion of the cause. Photos show failures involving transformers, fuses, breakers, switchgear, meters, busduct and other power system equipment. Lessons learned from these failures have been condensed into a handy checklist to improve electrical equipment maintenance and system reliability. This checklist is a valuable tool for facility managers and others responsible for design, maintenance or operation of critical facilities.

Speaker Bio:
Mr. Wally Vahlstrom is Director, Technical Services for Electro-Test, Inc. His responsibilities include energy management/auditing services, conformity/NRTL services, power system reliability work, and failure investigations. Mr. Vahlstrom has a BS-Electrical Engineering from California State University, Fresno and an MS-Engineering degree from the University of Santa Clara. He is a registered professional Electrical Engineer. Professional affiliations include: Institute of Electrical and Electronic Engineers, American Council of Independent Laboratories, National Association of Forensic Engineers, and Lightning Protection Institute.
